It's not really something that's overly complicated, you just have to work out the design and figure out how the pieces go together. I prototyped mine using paper, tape and staples. Once you're done refining the design, you create the final template using Illustrator (like I did), or Photoshop. For the stitching part, I used blanket stitching for sewing the pieces together and just some simple stitching for the embroidery.

The cases I've made so far are for iPod touch, although I do have plans to make them for iPhone sometime in the future.
Unfortunately, I don't wish to share the template I made as it's my own design. If you want to try making something similar, it might be a bit of a learning curve, but try and figure the design out for yourself.
