Zodiac Form (ゾディアックフォーム Zodiakkufōmu) is Kamen Rider Tuner's final form, obtained by using the Zodiac Raise Buckle (ゾディアックレイズバックル Zodiakku reizu bakkuru), given to her as a gift from the future by her supporter Monyu (モニュ Monyu, from Monyumonyu, the japanese onomatopoeia for crying/weeping), after she saw Misuzu wanting to help her love Raito/Kamen Rider Camino to fight Kamen Rider Onikuma and the Jyamato, but felt too powerless to do anything. With this new buckle, she can utilize several different abilities based on the twelve constellations of the western zodiac, such as super strength activating Taurus, poison manipulation with Scorpio and etc, and grants her the Zodiac Scepter (ゾディアックセプターZodiakku seputā), that can amplify her cosmic abilities to ranged attacks.
