I remember going into heartless form while fighting the army of heartless and nobodies, I got lost and couldn't tell where Sora was anymore.

That's Anti Form From Kingdom Hearts II. You into it randomly if you select any form but "Final" and have enough Anti-Points that the RNG hits a number that will turn you into Anti Form. Anti is completely melee based, no Keyblade. Also, it consumes ALL of your Drive Guage regardless of what form you chose.
Here's how Anti-Points work (from the official Bradygames guide)
Go into any form other than Final (or Anti): +1 Point
Go into Final Form: -10 Points
Go into Anti: -4 Points
Go into any form when an ally is unconcious (0 HP): +/- 0 Points
Obtain a new form (including Final, which is random): Points drop to 0
Chances of going into final form (based on points)
0-4 Points: 0%
5-9: 10%
10+ 25%
Odds Modifiers:
Scripted battles (story events): X2
Battles against Organization XIII: X4
Battle against Armored Xemnas: X10 ([goddamn...])
Battles with Non-Party Cooperative characters ([Such as Hercules in the battle against Pete): X0
