First off, I just want to say how much I love this art style. It's so simple and detailed all at once. It's sketchy, but in the good, intentional way. I don't see any problems with the lineart, either. It might look a bit better if the lineart was a bit paler or a softer colour, like a dark brown, but that's just my opinion.
As for the anatomy, I don't see any big problems. Actually, I was a bit surprised by this; most DeviantArt creepypasta fan art tends to mess up the anatomy pretty badly. The style goes so well with how you drew the character.
The blood on the axe should be more concentrated toward the tip of the blade and it should be a bit sharper, but these are the only problems I see. The shading and lighting are perfect and go so well with the style. The atmosphere is very dark and fits well with the Ticci Toby story.
Overall, this piece is amazing. That's all I can really use to describe it. Keep up the great work!
