In Tarot situations where sacrifice could be needed are indicated by the hanged man. At its highest level it could indicate Christ on the cross. at a more mundane level you might need to get rid of something like smoking or give up something to help someone else. I say this because I can see your picture with the women reversed as the hanged women.
This would make an interesting tarot card.
