First thing, I'm a girl (a little crazy, but a girl after all)
It's a pixel art icon... Exist many programs to do pixel art, but I use Photoshop.
First I create a new document: size 50 x 50 pixels - 72 dpi - RGB colors (Use the size you want, but with this size, you can use your icon in Deviantart, as avatar or emoticon)
There are many tutorials to learn how to make pixel art. I recommend you see this first, because it is very similar to what I do PIXEL ART TUTORIAL AND STUFF by
When you're done, go to "Menu / Save for Web & Devices..." , and save the document as *.gif

Want to transform the gif to ico? The program I used is obsolete, so you have to find a way for yourself.
