The colors are exact hexadecimal triplets, taken from the team's official logos, from their official websites.
Some teams, the Canucks, for example, have 2 patterns because they had 3 or more main colors on their logo, and, as I am a Dallas Stars/Minnesota Wild fan, I don't know their 2 main colors. DDDDD:
