Maximum of 6 points per stat.

You can put in a picture via editing software such as Gimp, Paint tool SAI or Photoshop!

You use a program like Photoshop to fill it in
Naturally, the .PSD can be opened with other programs as well.

Nah,I think I realized what the problem is.
Sai cannot read all the special tricks used on Photoshop and my Photoshop,Elements 10 does not open folders....for unknown reason. :I

Nah,It's okay.The only thing Sai really has problems with is the black background of the info itself,which I was able to solve with the Water-pen and the start-chart and about that,I dunno,let's see

Well,besides these two problems there are actually none.
The Start Chart on Sai has the problem that only the six lines from Border to middle are there the other lines are just represented as while fields on different layers.

I really like the looks of the registration form, good job on that. ^^
Oh, also: do you mean with "Family" the Field which the Digimon is in? Like Dragon's Roar, Virus Buster, Nightmare Soldiers and so on? Also, should we write the height in centimeters or inches? or whatever it is called, I really think you should add this information somewhere.

In photoshop, you can select and hide the female option and replace it with the male option.
