i probably sound like a retard.. but how to you use it?

You download the image, save it, then re-open it into your art program just like you would any other file. Then, in whichever program you're using- draw over it/alter it with your own pixel art so that you customize the base to however you'd like it. Sort of like drawing over a doll to add on clothes, hair, etc.
