Exactly what the title, and inner descrption implies.

This is a meme for folks who either have their own other-worldly character who acts as an eldritch horror/cosmic horror, or has an interpretation of an existing goldy character who has another form beyond the actual one they already have. Or is simply adopting a base form in order to hide their true powers, much like Frieza himself once did when we first met him, and his ability to transform. There is almost no limit to this idea.

So if anyone has a character they have who has two forms, or is a godlike being who exists on a separate plane of reality like Stephen King's IT, and has both a base form to better blend into its environment, and a form which is actually only a close second to the real thing because what we see as its true form is only what our minds are capable of preceiving of it, then feel free to put them in those two boxes. You can fit more than one pic in the "physical form" box, but the true form box must remain singular. After all, there's a reason it's called the true form.
