To be fair, that doesn't appear to have the same effect in Equestria, as Fluttershy broke a bear's neck and that counted as part of a massage.

Bones ain't the problem. Breaking someone's neck in the real world involves snapping the spinal cord (the one that is a bundle of nerves that connects the brain to your body). This effectively cuts off all communication between your brain and every other vital system in the body. Death follows pretty quickly. In Equestria, spinal cords are apparently made of more flexible stuff because that bear's neck went all the way around and that would DEFINITELY have killed any real bear/human/pony.
