Yuji and Sukuna were lovers in the Heian era, when Sukuna split his humanity off to try and become more powerful*. A thousand years later, after being ripped apart from each other, they reunite. Yuji eventually does accept that he's the reincarnation of Sukuna's husband, and indeed has the memories of his past life, and the two finally couple once again in an onsen after escaping the chaos of the war for a brief moment.

*Jogo says that curses are the 'true' humans, as they are made from the purest emotions, while humans use happiness and deceit to hide their real feelings. The veracity of this claim is highly debatable, but my person headcanon is that Sukuna, who already has a lot of fuckery going on in just what he is and how he's not totally a human, and has never been treated like one, would absolutely cut off the parts of him he sees as being 'most human' to become stronger in the sense of a cursed spirit. If cursed spirits are the rawest human emotions--hatred, fear, hunger, pain--then to become as strong as a curse one would need to remove all the 'false' emotions that makes a human, a human. Sukuna did so, via a sacrificial ritual.

It made Yuji. Spawned into existence as a young man, basically everything good Sukuna wanted to discard about himself. (Ryomen Sukuna means two-faced fiend, he was immortalized as a monster with two faces, one evil and one kind, in the Nihon Shoki and other texts; Sukuna is the unkind face, Yuji, literally meaning kindness, is the kind one) The two found each other, as they were not meant to be apart, and had the same pull to one another as twins do in the Jujutsu world. (Sukuna himself is an incomplete twin-absorbing-their-other-twin-in-utero caused by a failed abortion but that's neither here nor there) Eventually, they fell in love. When Sukuna realized that his softness and affection for this young man came from the fact that they were two parts of a whole, he was pissed and wanted nothing to do with Yuji...but Yuji won him over a second time, because Yuji is nothing if not stubborn.

In present day, Kenjaku used Sukuna's final finger to create Yuji, which was perfect for his soul to reincarnate into--because remember, he and Sukuna are one soul. The rest of that soul (Sukuna proper, all 19 other bits of him anyway) were drawn to Yuji like a moth to flame, two opposing poles of a magnet. You cannot separate two halves of one thing and expect them to ever feel complete.
