Magic doesn't work like that. Even Unicorn Magic. She gets slightly tired when she uses big spells. A healing spell would take a LOT out of her. Especially one of that magnitude. Considering she was almost dead, was a couple times in fact, using the healing spell probably would kill her. Even in the real world, magic takes a toll. The stronger the magic, the higher the price. And yes, there is magic in the world, you just need to know where to look.

Having undergone major surgery myself I can tell you that that soon after a surgery of that magnitude the pain meds Twilight would be on would most likely put her into a medically induced comma. Even if she was awake she'd barely be able to talk let a lone think straight.

When I woke from surgery in a druged state my first words were "ouch" and "can I get a blue slurpi?"

That's some good medicine they've got in Equestria. All doped up on painkillers and still totally lucid.
It's a shame, really. The effects the anesthetic had on my mind when I got my wisdom teeth out were amusing; definitely the best part of the process. Much better than pain and not being able to chew properly. I'm sorry she's missing that.
