Yeah, the pixel art isnt that bad, is it
I'm not sure what do do with it though...it's not quite easily enough recognisable.
Oh well. I might go back to it at some point.

I think you should draw the lips too, even if they are very pale in the original picture. Maybe it's just me, but I had a hard time figuring out it's a tongue, because there are no lips. Otherwise good work.

Well, The syringe looks good, The tongue isn't bad but as you said it needs abit more shading.
And of course you will get rid of the white bits. good job so far.
