It began with a plane, extruded on the edges to make the basic shape. Then turned to a subdivision mesh (catmull-clark) to smooth the edges. The sharp edges are made using extrusion without translating the new face, so they are not affected by the subdivision algorithm.

I used the blender built-in raytracer with ambient occlussion turned on (both add and substract operations). I don't remember the exact settings, but that usually is a matter of trial and error. I only used two lights, a sun directly into the plane and a lamp that only casts shadows but does not illuminate.

wow, it looks really good. I thought you used somthing like Yafray or somthing. You did really good on the lighting too for only usin 2 light. Very good job!
