The Gimp for creation and HCAMit for 3D preview.
It's not making it any easier that I must save the work, load it up in HCAM, render, note corrections needed, apply corrections in GIMP and again, save, load, render, note corrections... Takes ages.

HCAMit, [link] a CAM projecting software. While it's a decent program for projecting technical pieces (machine parts etc), it absolutely sucks when it comes to creating artwork from scratch - it's an engineer's tool, not an artist's tool. But it can import a heightmapmap .bmp picture and generate a 3D preview of it. It can also generate machine tracks for manufacturing a sculpture basing on the heightmap.
The GIMP, [link] is a general purpose 2D graphics program, for art and image processing - a'la Photoshop. It has no 3D facilities whatsoever, but it can export uncompressed grayscale .bmp, the kind HCAM reads.
So I can draw a piece (any piece) in GIMP, then export is as grayscale bitmap, load it into HCAMit and see the 3D preview based on the .bmp. Lots of "smudge", "blur", "airbrush" with very low opacity, "colors->curves" tool combined with linear gradient, a picture/photo in the background for reference, and lots and lots of patience, because as I draw things in GIMP, the results are not immediately obvious - I need to draw what I "feel" should seem right, then save, then preview in HCAM, and then blindly apply any corrections over again.

well.
i'm an engraver too.
i speak english not very well.
but:
i've downloaded th gimp,i't's like photoshop but i haven't understand how you arrive at a height map from a simple draw.

Usually, the process goes like this:
- pick a source drawing - preferably lineart without shading/filling/etc, just contour. Load it.
- Image>mode>grayscale
- add layer, fill the new layer with black.
- set the new layer opacity to about 80%, so that the original image shows through.
- paint with airbrush tool, smear, blur tools, whatever, to get the graymap - simply draw a picture like the ones I posted. You may want to set opacity to 100% before saving, or leave it at 80-90% and elements of the picture will be transferred into the greymap (see [link] )
You may want add another layer - filled with white, 20-30% opacity, on top (don't edit it and remove it when finished), to create the darkest/deepest parts (in this piece: tail of the unicorn, the right pair of her legs) - they are very hard to see normally, and the extra layer makes them medium-gray, way easier to see.

It's intended to be fed to 3D rendering software, 3D printers or CNC milling machines, to produce actual 3D pictures.

Well, it's blueprints, not the final work. Like reading musical notes instead of listening to music.
