Description Greetings:

I have created this "inmate registration form" for myself - well, pretty much all there is to know about me, at least - for the inclusion to this wonderful little spot on dA known as It's a place where the nightmares take shape and the shadows whisper.

Heaven help me, I've held conversation with them for too long in my own little gas-lit corner.

A note of explanation: A "medical holiday" is something of a medical relic. Once upon a time, an "asylum" is the equivalent of a long-term nursing facility similar to a nursing home or a convalescent home nowdays. A "sanatorium" is where you put away the mentally unstable for treatment. An "asylum" is a place where you check yourself in to give your mental state a break. The doctors and nurses treat you for "neuraesthenias" - grief, burn-out, mild depression, mild anxiety, hysteria... a large number of complaints.

So, as for yours truly... perhaps it's a good thing that I'm entering an asylum for an extended medical holiday. This world at this time that I am living in can take a toll on me.

You mention electroshock therapy in your form... have you ever had ECT? ECT is short for electroconvulsive therapy, just in case you were unaware, otherwise known as electroshock therapy. You don't have to answer if you don't want to, I'm just curious. See, I have an ECT appointment on friday, and it's always nice to find others who have experienced the same thing.

I most certainly did mention ECT in my form.
And I am certainly familiar with electroconvulsive therapy. Unfortunately, I was never a recipient. I am the prescriber.

In the world outside of MMA, I am a medical school graduate who has spent some time on both sides of the psychiatric ward. I had serviced in two of them and I was almost an inmate in one. During my stay, I have had some experience on the -other- side of the ECT treatment. From what I do know of the recipient side of ECT, it can be an unsettling experience. The general convulsion is definitely no longer part of the treatment (we usually inject our patient with a general muscle relaxant right before the treatment). But I have to confess that I do not know what happens in the patient's mind during this forced re-boot. I am very interested in talking to you about this procedure, if you are still willing to converse with me.

Wow, this is a late reply.
Do you know Dr. Welch? He's usually the one that preforms the ECT at MGH. Anyways, yep, they use succinylcholine as the muscle relaxant, at least on me. I was supposed to have an ECT treatment yesterday but I was vomiting every time they moved the bed so the anesthesiologist said no (it was the remnants of a sinus infection). Instead, they pumped me up with Zofran and a liter of fluid. I'd be absolutely fine with talking about ECT. I'd love to know what it's like from the other side.

I'm just glad that you decided to reply back, you know.

There's a small group of physicians around the world that still prescribes ECT. While I don't know Dr. Welch personally, I am sure my teacher Dr. Milev does. ECT has been very badly maligned in popular media, and yet it remains one of the fastest and cost-effective way for people to get out of deep, deep depression. ECT has been modified quite a bit since the 70's (but most folks' understanding of ECT still remains in that primitive stage.) In fact, since the advent of psycho-active drugs, a lot of psychiatrists feel uncomfortable prescribing ECT nowdays. (Here's an interesting - but sad - trivia: Doctors in Slovenia will prescribe ECT to their patients, but there are no doctors in the country who will actually preform it. The patients have to go to the neighbouring Slovakia to get the treatment.) This also means if we try to suggest ECT as a treatment to our patients, many of them refuse.

I have done some research about how ECT works, but the truth of the matter is that we are still not sure about it. But we know it works and it is relatively safe. If you ask me... a group of my friends and I would choose ECT over the meds if we ever fall into the state when we need treatment.

I'm sorry to hear about your sinus infection, however. Are you getting better?

I'm much better now. I had ECT on friday and it went fine. Apparently my heart rate was up to 150 bpm when they plugged me into the monitor. I get really anxious whenever I go in for ECT because of that time I was "aware" (I guess that's what they call it). Well, at least they don't do insulin shock/coma therapy anymore.

I'm so very glad to hear back from you, even more so to hear that you're doing better!

I think you've bee somewhat aware when you underwent your ECT before. You remembered some part of it even if you didn't recognise that you remembered anything. That sort of experience can be stressful - and your body reacted to the stress. (A similar thing is called "White coat syndrome" where people associate meeting with the doctor with stress. When they visit the doctors - or see anyone with a white coat - their blood pressure rises despite their conscious mind knowing that it's OK. White coat syndrome is actually very common and folks have been mistakenly diagnosed with high blood pressure because of it in the past. Heaven knows... I've got a bit of that myself...)

Let me let you in on a bit of a secret: there -was- a rationale behind insulin therapy. ECT was the first treatment people tried that had some very dramatic effect in treating intractable depression. So the enterprising doctors associated a brief loss of consciousness as a kind of "hard reboot" system for your brain. They think this rebooting mechanism is what's making the patients feel better when they wake up. Therefore, when they realised that insulin could make people pass out, they tried it on psychitric patients as a safer alternative to ECT. Well... boy, were they wrong. Not only is it not safer, it is also ineffective. Which is why I can't agree with you more - thank goodness we have totally abolished these so-called therapies. They have their space in medical history (just like using mercury to treat syphilis) but... thank goodness they are gone now.

"Sinistrose" means "left-handed sugar". "Dextrose" means "right-handed sugar".
We're technically one and the same, and yet there is a very distinct difference between us. I sometimes wonder who is the one that lives in the mirror and who is the one that lives outside of it. But that's why I carry out conversations with him, you see.

But that's enough of that. I think I've heard from him too much lately. That's why I am here on a medical holiday, right? And I did say to the grounds-keepers to keep mirrors away from me. I think I would like to live without having to listen to my mirror-self for a while.
