This infection AU is inspired by the Rage Virus from 28 Days Later and the symptoms are the same as the original, but more realistic and a slow progression.

Backstory:

The virus was created in a lab mixing the Equine Ebola Virus and neurochemicals to try to create an angry inhibitor. But to test the new inhibitor, they needed subjects and they used "volunteers" to test the inhibitor. But the inhibitor doesn't work right and it caused the subjects to get gravely ill and have uncontrollable rage instead. An unauthorized pony entered the lab and saw the horror going on in the lab, as a result, they arranged a rescue party to set them free, but what they didn't know was, the subjects were in the late stage infection and they were no longer the intelligent ponies they once were, instead being reduced to a more primal and feral state.

On the first day of the outbreak, the rescue party is assembled and they break into the lab. The scientists warn the ponies that the test subjects are unsafe and infected, but the group refused to listen and manage to get into the lab. The ponies, having not listened to the scientists in the lab, set the infected ponies free, instead of the subjects leaving the lab in a hurry, they attack the rescue mob and the employees in the lab, a survivor of the incident describing the test subjects of being uncontrollably angry and completely insane. This incident was the start of the outbreak as the infected did leave the lab to infect several others.

Within days of the outbreak, the Princesses advised the non infected civilians to leave their homes and go for the destinated shelters. They assembled a huge army of ponies experienced in combat (The royal guards, the Equestria military, the Wonderbolts, and basically any pony that has battle experience) to keep the healthy ponies safe. Security at Canterlot was maximized and Princess Celestia enforced a protective shield to keep the infected out. Princess Twilight Sparkle made it her job to find a cure and vaccination for the disease and founded a secret lab project in the protective shield of Canterlot. She hired ponies and creatures experienced in alchemy, magic, engineering, lab tech, and medicine from all over Equestria to help find the cure/vaccine. But to create the cure successfully, they needed live subjects so Princess Twilight sent out capture parties to catch the infected and then take them into the lab. Ponies who encounter the infected are required to hazmat suits so that they can keep themselves safe.

But despite the shelters being permitted, not every pony was able to escape to safety and instead, try to survive and basically avoid the infected at all costs. A lot of ponies went into lockdown upon learning about the infection and many of them went as far as putting wooden planks to keep themselves safe. Because of this, Applejack, Rarity, and Rainbow Dash organize their own shelter and rescue party to help survivors, so far they've been able to find quite a few survivors. But Fluttershy on the other hand was nowhere to be found and was presumed to go into hiding and locking down her cottage upon learning of the outbreak. Pinkie Pie got attacked on the first day of the outbreak when a group of infected escapees come to Sugarcube Corner. Pinkie noticed something was very wrong and tried her best to fight them off, but no matter what she did, they were just too powerful and she got bit in a few places. Prior to this, Pinkie's Pinkie Sense has gone haywire and she was having uncontrollable twitching, bouncing, and etc, but she couldn't figure out what was wrong and went frantic. Her friends did believe her but being clueless like she was, they were of no help, Twilight would later regret being so dismissive despite having known Pinkie's 6th sense for years at this point.

About the original virus:

Equine Ebola Virus works like the Ebola Virus in real life. The virus attacks the cells in the body and liquefies the infected patient from the inside out. First symptoms start out very flu-like, headache, body aches, chills, fever, and sore throat. The next stage brings vomiting and diarrhea, quickly followed by bruising. At the later stage, the individual starts to have hemorrhaging from all over the body. Blood vessels breaking in the eyes, nosebleeds, bleeding in the respiratory tract, bleeding from the intestinal tract, and basically everywhere. The ending result, without treatment, is almost always death. The individual usually dies from organ failure and blood loss due to the virus destroying the cells in the organs. The incubation period for the virus is between 2 days to a week. With treatment, the individuals start to recover 7-14 days after symptoms, but otherwise, it results in death. In ponies, the main transmission of the disease is from primates and bats, but only with certain strains. It originates from the land of the sub Saharan deserts of Zebrica. Most strains only affect those animals, but some strains affect equines of all kinds (horses, zebras, ponies, donkeys, and miniature horses (pygmy ponies).

The Rage Virus basically resembles the classic symptoms of the Equine Ebola Virus, but with mental symptoms combined. The disease has four stages. The infected live on average about a month or two once contracting the infection. The infection is almost 100% fatal once symptoms start and the only way to save a pony from symptoms is to amputate the limb that has the bite site. Cause of death in the infected is dehydration, starvation, and organ failure. The incubation period for the virus is 3 days to up to a month, but less than a week on average.
