Maybe change the last effect to something like this...
'Remove X +1/+1 counters from Polypus Cloaked Assistant: Look at the top X cards of your library, where X is the number of +1/+1 counters removed this way, then put them back in any order. Draw a card.'

I think what they are getting at is that according to state-based effects, if you remove all the counters, the creature has 0/0, which triggers state based sacrificing, which bypasses the stack.

It might have no +1/+1 counters, but some other effect (Glorious Anthem, Giant Growth, etc.) may be boosting its toughness.

try not to use set symbols from actual sets
Going odd the flavor text, the third ability would be 'sacrificeΒ

the problem is that sacrificing that way don't allow to look at the number of +1/+1 counters, may be i will find a better wording later
