Well, actually, it is possible to "go wrong" with ball gags. Truth is, they are a very specific tool and good for only limited circumstances.
First, there's no way that a ball-gag alone can silence a damsel. Vocal cords are of course unimpeded, and there's plenty of mouth resonance behind the ball to generate a solid amount of volume. Next, ball gags don't completely impede word articulation. Some phonemes get replaced by others, sure, but the damsel's language would still be reasonably understandable, because the tongue can move around. So forget all those fakey movie and TV scenes where the use of a ball gag is promoted as hugely effective. It ain't. It's there for show, for intimidation, and for humiliation.
Which leads me to why one might use a ball-gag, especially in the situation and scenario in this image: it's mostly for the aesthetic pleasure of the dommes, and for the humiliation of the sub. Ball-gags produce lots and lots of drool. They also prop open the jaws and prevent closure even when closure becomes the most important thing in the world to the DiD. Why? Because muscles and ligaments get sore, that's why. But if this is an interrogation scene, with the bound au pair being forced to answer questions put to her despite all the slurring of her articulation and the drooling, etc. etc., then the humiliation factor and the remaining capability of communication make the use of a ball-gag reasonable.
Alas, the fantasy of ball-gagging a captive becomes completely untenable when the ball itself is as huge as this one. In place for any longer than a couple of minutes and the result is likely to be a very bad case of TMJ at a minimum, and a dislocated jaw as a higher risk. The sub will be in so much pain that it won't be sexy, won't be a focused experience, and will simply end up in tears and even screaming in pain. That's when you know you've moved entirely out of sexuality and into power-fed torture.
Suggestion, therefore: forgo the fantasy aesthetic of the ball gag and use a panel-gag instead, one with an interior-residing leather plug that fills the mouth cavity, and is attached to a leather or vinyl panel that fits tightly over the mouth opening. That combination makes for complete inarticulateness, and is actually a better volume muffler.
A slightly different suggestion would be to pack the girl's mouth with some kind of wadding, and then secure that stuff with a much smaller ball between the teeth. That would produce enough muffling and inarticulateness, while maintaining some of the ball-gag aesthetic. But be careful: too much wadding pushed too far back into the mouth can Β impede airflow, and you'd quickly have a dead sub to deal with.

The main thought behind the characters are the two older ladies are very rich couple, who take on some au-pair to help them run their big house, but things don't quite as planned for the au-pair!
