Theory time.
The Auditor has been behind the madness all along at least since Madness Combat 3: avenger. When that fool of a Sherrif activated the improbability drive, it allowed the Auditor to basically leave hell, constructing better improbability drives so he could harness the powers of hell onto the mortal plane, distorting reality. In addition, also using his technology to engineer and enhance humans to become his personal supersoldier army.
As for why Hank was targeted? He was already in hell and got out when he was given a second chance. The Auditor is trying to reclaim him.
Now, the theory of Madness Combat 11.. Tricky did indeed nab the secret inprobability drive which is more potent than anything his own portable drive could do. Complete with the halo, it allowed Tricky to take over hell. The Auditor was still connected to the Secret Drive though, explaining why he did not die, or perhaps did die, but simply rendered his death invalid.

I wonder what were doing those AAHW in "hell". Were they earlier-sent expedition? Or Auditor used his remaining power to spawn copies of AAHW models he knew?

Theory Time: I think the purgatory is how the improbability drives work , i believe each one of them have a pocket hellish dimension or have access to a particular one, in this dimension the laws of reality don't apply, and what the drive does is merely invoking a part of a dimension into reality making all sort of bizarre stuff to happen. is also where most of the buildings and mooks are stored (and why more often than not buildings fall out from the sky, its the drives that invoke them from this hellish dimension). probably the auditor is the creator/owner of this purgatory/hell.
