It's actually done with the same concept of a bed of nails. The more hooks you have, the more evenly your weight is distributed across those hooks and it's supposed to be less stressful/painful. The fewer hooks, the more weight is on those few hooks and then it's supposed to hurt more. BMEZine.com is awesome for looking up things like Suspension.

Oh, wow! I've seen something like that before! In New York, these people can dangle from giant hooks pushed into their backs for a minute or so. I imagine it's pretty painful.

I don't think it's very painful. The flesh on your back isn't very sensitive for beeing pierced...
Everybody can handle the hooks you mentioned. You can even pull pretty hard on them...
