It's impossible for it to be real...look at the light sources, the highlights say they come from both the left and the right, but the shadows cannot look that way if that were true. If that's not convincing enough, so many tiny details are exactly the same on each side. Plus, I don't think she could have a head.
But I don't care, because it is a gorgeous image! It certainly makes one think. Amazing contrast and tonality. I wish I could get images like that...

I'm not sure why I'm saying this, because you posted that comment ages back, but if you look carefully there are little differences between left an right.
Just an example there are a few little dimples on their right arm, and the veins on that arm stand out a bit more.
