Description Having won the "heavyweight battle", Dobinxette is promptly led into what was once her private quarters, now converted into a massive dining room with a room-length table covered in food. Dobinxette is seated and quickly helps herself. Despite rapidly guzzling down far more food than she should normally be able to, she starts to feel unusually woozy, before promptly passing out face-first into a huge slab of hot-sauce-covered meat. After getting no response from her, the duo and Shepa decide to investigate a strange reaction in her cranial region, arriving in the Cortex Command. As they explore the area, they encounter several new enemies.

Birdbrains are a new kind of flying enemy, rendering them immune to ground-based attacks, such as hammer strikes. They attack by diving at one of the duo, requiring the target to hit them away with a hammer counter. Sometimes, they will split in two, before attacking both of the duo, either one after the other or at the same time, requiring both characters to defend themselves. Both variations can trip if they hit. Alternatively, the Birdbrain will quickly swoop down in a loop at one of the duo, requiring them to jump over or on to avoid or counter respectively. After attacking the first character, they will fly back up and over the other, requiring them to not jump.

Synipers are another new enemy, which attack from range with a single shooting attack. The position of their targeting recticle indicates how the target must defend themselves. If it is aimed at their head, the Syniper will shoot a larger but slower blast, which the target must destroy with a hammer counter. If it is aimed at their body, the Syniper will instead shoot a smaller but much faster blast, which the target must jump over to avoid. If the blast hits, it can deal the 'DEF-Down' status effect. Sometimes, after firing the Syniper will quickly hurl itself at the other character, requiring them to hit it away with a hammer counter.

Cerebrelenos are a new variation of Seleno, and attacks using the same ramming attack. It can either charge at one of the duo, requiring them to jump over or on it, through the duo one after the other, or it can clone itself before attacking both at the same time, or one after the other, requiring them to both jump in a matching pattern. If a charge hits, it can trip.

Whilst exploring the area, the duo can learn the Guess-Who Gun special attack, a new multi-target attack that makes up for a lack of precision with versatility, being able to target both spiky and airborne enemies for decent damage. Upon loading the cannon, four copies of each of the duo will be in the eight chambers, and their positions must be remembered, as each chamber is fired in turn, in a chosen order, since the chambers can be rotated freely.

Eventually, the duo arrive at what appears to be a major nerve leading to Dobinxette's brain, and come across none other than Misty herself, or rather a projection of her, attempting to hijack Dobinxette's brain and take control of her too. Realising the disastrous implications should they lose the cooperation of their host, they move to intervene, actually surprising Misty, who explains that she did not expect them to survive their little voyage into Dobinxette's belly. Then again, the plan did lack 'precision', and Misty decides not to rely on outside factors and battles the duo herself...
