Worst I've ever seen personally was a guy who was driving with his left foot out the window against the wing mirror, and he had a trumpet in his right hand that he was playing. On the highway. 0.o

I honestly don't know the guy's story. I was driving back from a long work day, well more specifically getting a ride back from a co-worker (taking the subway involved going the long way through Manhattan and took some two hours) when I saw the guys foot up on his mirror. As we passed I saw what I was pretty sure was a trumpet, or at least something similar looking and holding it up like he was playing.
