I'm sure there are some tutorials about this already but I thought I will make one.
Since I've done Anxieto and he have a knife normally attached to his right leg, I have troubles on how to put it in PMD/PMX Editor the right way so the bones won't get mess up.
But nothing worked before I remembered it can be done in MMD itself~
The function is called Outside Parent and can join/merge the bones of two different models with each other!
I hope this will be helpful to you as well. ^^
