This is step 2) "How on earth is this (pregnancy) possible?"

Meet my story's -Uunu Styx- parasite. This thing slithers much like a snake, but is able to latch fairly well to things with those two appendages.

It closes in on calm warm bodies (something/somebody sleeping) and locates the reproductive area. It then uses those appendages to insert a numbing and temporarily-paralyzing agent.

It's at this point that it punches it's way into the nearest open space near the reproductive organs. In females, it will simply punch into the vaginal canal. In males, it will punch in between the penis and anus.

Once inside, it is large enough to stretch from the entry point to the stomach in most bodies. At this point, it uses the unfortunate victim's body as a coocoon for itself, and begins transforming quickly into an entirely new reproductive organ. Attaching to the nervous system and circulatory system.

Then over the course of up to a year it will absorb DNA from it's host, and begin growing a near-exact clone of the host's body inside itself. The host will find him/herself nearly completely drained, nothing but a shell, at the time of birth. And it is the birth process that leads to definite death of the host.

What is born is the near-exact, fully grown clone of the host's body. This clone will suffer from amnesia. The only difference in this clone's body and the host's body is that now the parasite's reproductive process can truly kick in.

Immediately the clone body will be capable and -forced- to breed. If not given fertile DNA from another person's body, the clone will simply birth more Uunu Styx parasites.
